Animals can only be seen if referred by a veterinary surgeon. Please complete the information on the referral portal. If you prefer, you can download the referral form and email it to us at info@eyevet.ie

Only procedures relating to the eye condition will be carried out. The animal will be referred back to you for all other procedures unless you request otherwise. Cases will be returned to you for follow-up care where this is appropriate.

If you require a second opinion rather than complete care for the eye (for example, an opinion as to whether the eye may be saved or not), please indicate this on the referral letter.

When referring, please include the following information:

• Owners name, address and mobile telephone number
• Animals name, signalment, history and current medications
• Any other relevant information
• Please indicate whether the referral is routine or urgent
